To file a Complaint with an Agency or just to report any Complaint to us to keep on File with us, we must have all of the following information from you: 
 
In the Subject Line put the name of the Agency and the City and State of the Agency and what it is you want either, REPORT to just have it on File with us or COMPLAINT meaning that you want us to forward to the Agency what you have a Complaint about.
 
In the Message part explain in detail what happened to you, or what you want to see changed in the Government or any of the other Agencies that are listed above. Include who was involved, the names of individuals involved, the names of the Agencies that are or were involved with your Complaint or Report and their E-mail address, the address and phone number of the Agency and the address where the incident happened. Also include the time and date that it happened and the names, addresses, E-mail address and phone numbers of any Witnesses who may have seen what happened or can be of help in regard to what happened to you or what you want to see changed in the Government or other Agencies listed above. Include your name, address and phone number and a contact phone number, address and E-mail address of a family member or friend who would know how to contact you.
